Registration
You can register with us if you live within our Practice area.
This includes Balmullo, Blebo Craigs, Boarhills, Ceres, Dairsie, Dunino, Guardbridge, Kingsbarns, Largoward, Leuchars, Peat Inn, Strathkinness and St Andrews.
Here’s how
Step 1: Email us your Registration form and Health questionnaire
- Download your Registration Form: GPR-Scotland-2024 pdf form
- Download the best medical questionnaire for you:
- Pipeland Student Health Questionnaire– For University of St Andrews students
- New Adult Health Questionnaire (PDF) – Excluding students at the University of St Andrews
- New Child Under 12 Health Questionnaire (PDF) – Registering a child under the age of 12
- Fill out both forms on a desktop, laptop or other device with a PDF editor such as Adobe. (Or download free PDF editor to your phone from https://www.apple.com/uk/app-store/ or Android Apps on Google Play)
- Email both forms to Fife.registration@nhs.scot
If you don’t have email, print both forms and return them to the Practice. If you can’t email or print, call the practice to discuss.
* Unless your forms have been filled out incorrectly, we will not respond to your email and you can assume your registration is complete within five working days.
Step 2: Sign up for repeat medication
If you have repeat medication, you can sign up to our Online Services 28 days after your registration has been completed.
